The problem

CCM revenue is on the table.
Most practices can't capture it.

Medicare pays primary-care practices about $65 per patient per month for chronic-care management — if you can prove monthly contact, logged time, and a current care plan. Most independent practices either skip it and lose the revenue, or outsource it and lose the margin and the patient relationship.

01

Time tracking is manual

Care coordinators stash time in spreadsheets, sticky notes, or chart templates that auditors won't accept. CMS wants 20+ minutes per month, documented precisely.

02

Care plans drift

CMS requires a current, patient-specific, comprehensive care plan. Plans copy-pasted from the last visit don't pass audit — but writing fresh ones takes hours per panel.

03

Billing eligibility is opaque

Knowing who is billable for 99490, add-ons, or complex CCM at month-end means checking every chart by hand. Most practices guess, undercode, or miss the window.

How it works

A CCM workflow built from the exam room out.

Empower Medical is what we use at our own primary-care practice in Brooklyn. Every feature exists because Dr. Epelbaum needed it to run CCM correctly — not because a product team imagined a use case.

Time tracking01

A live timer, per patient, per encounter.

Start a timer when you call a patient or open their chart. We log the minutes, the activity, the channel, and the staff member — audit-ready out of the box.

Medication reviewBP coachingSpecialist coordination
Care plans02

Care plans that match the month being billed.

Build a current, patient-specific care plan in minutes. The matching-month plan travels with the billing note so the signing provider sees it inline. Backdated billing scopes to the consent date, not enrollment.

Billing readiness03

Know who's billable before month-end.

See every enrolled patient's CCM status in real time — minutes, contacts, care-plan status, and eligible CPT codes. File compliant billing notes with CPT codes, ICD-10s from active problems, and the care plan attached.

EHR sync04

Integrates with your EHR.

Syncs demographics, problem lists, primary physicians, and staff attribution. Time logs land in the chart credited to the staff member who logged them, and visit notes arrive with their billing codes filled in.

Built by a practice, for practices

We needed CCM software that didn't exist. So we built it.

Empower Medical Primary Care PLLC is a primary-care practice in Crown Heights, Brooklyn. When we set out to deliver chronic-care management to our Medicare patients, the options were bolt-on chart templates that fail audit, or third-party vendors that take half the revenue and own the patient relationship.

Neither felt right, so Dr. Epelbaum and our team built our own. We've run it on our own panel since 2026, and now we're opening it up to other independent primary-care practices that want to do CCM properly.

“The most important medical work happens between visits. Our software just makes that work billable.”
— Dr. Mark Epelbaum, DO
